Long Term Care Hospital

A Special Hospital Meeting Special Needs

Noland Health Services' Long Term Hospitals (LTHs) are designed to meet the special needs of patients suffering from chronic diseases or complex medical conditions that require hospitalization for periods that generally exceed 25 days. Patients, who are too ill to benefit from services provided by rehabilitation or skilled nursing facilities, need specialized nursing care and technologically advanced therapies. LTHs provide an interdisciplinary team approach to help patients attain optimal quality of life.

Our Hospitals Mission

Noland Health Services' Long Term Hospitals are regionally based referral hospitals dedicated to meeting the complex treatment and clinical education needs of our patients and families who require extended or specialty focused stays in a hospital setting. Our uniquely trained staff utilizes a goal oriented, outcome based, interdisciplinary approach to treatment designed to facilitate recovery and optimize our patients’ quality of life. We strive to serve our customers as centers of excellence through community education, defined customer programs, and the delivery of personalized treatment in a cost-effective manner.

Case Management & Quality Assessment

Our case management program includes patient screening, continued stay review, discharge planning, and post discharge follow-up. Our primary goal is to provide high quality, medically necessary care in an efficient and cost-effective manner. Nurses, and a member of the medical staff, assess all referrals prior to admission to ensure that prospective patients are appropriate candidates for admission. The patient's progress is monitored on a regular basis and discharge planning begins on the day of admission. After discharge, a registered nurse provides a follow-up review to identify any additional needs the patient may have.

Our Performance Improvement Program, based on Joint Commission on Accreditation of Health Care Organizations (JCAHO) and Medicare guidelines, is designed to continually monitor the quality of care provided and the utilization of services.

Long Term Hospital Services

Our direct patient care staff is skilled in the special care needs of long term patients. Most are Advanced Cardiac Life Support (ACLS) certified and experienced in treatment of chronic diseases and critical care protocols.
